Mazda
North American Operations (Mazda
) is recalling certain 2004-2006 B-Series vehicles that received replacement front passenger air bag inflators under a previous recall. The replacement front passenger air bag inflators may have been installed incorrectly.
Remedy
Dealers will inspect and reinstall the front passenger air bag inflator, as necessary, free of charge. Owner notification letters are expected to be mailed January 12, 2024. Owners may contact Mazda
customer service at 1-800-222-5500 Option 6. Mazda

Motor Corporation has decided to conduct a Safety Recall on certain 2004-2006 B-Series Trucks affecting 9,909 U.S. and 2,832 U.S. Territory Vehicles. Please see the description, model, year, and VIN range below for the vehicles in this campaign. Additionally, 2,801 vehicles sold by Mazda
Canada, Inc. are also affected by this recall and should be repaired if presented to a U.S. Mazda
dealership.
Concern Outline: The replacement frontal passenger air bag inflator may have been installed in the incorrect orientation during the previous Takata recall repair. The recall repair instructions may not have been followed properly, causing the inflator to be mis-installed. Vehicles in Safety Recalls 181 ?Land 2318A in Open status are not affected by this campaign.
Repair Outline: Dealers will inspect the passenger side frontal air bag inflator orientation. If needed, the existing inflator will be re-installed in the correct position.
Chronology of Defect
February 21, 2023: Mazda
received notice from the Ford Motor Company regarding the possibility of a mis-positioned passenger frontal air bag inflator. This may occur during completion of the recall remedy intended to resolve a potential air bag rupture from the original Takata inflator. Ford discovered their technicians did not follow the recall repair instructions completely.
Note that Mazda
’s MY2004-2006 B-series trucks are manufactured by Ford Motor Company and share the same platform as Ford’s MY2004-2006 Ranger trucks.
February 22, 2023, to March 1, 2023: Mazda
began internal discussions to determine assessment of field conditions.
March 2, 2023, to April 30, 2023: Evaluations were conducted based on available data; collected and reviewed internal data, repair instructions, other vehicles.
May 1, 2023, to May 15, 2023: Continued assessing field conditions from available sources and other affected vehicles.
May 11, 2023 : Mazda
modified the warranty claim process for active recalls 1817L (17V806) and 2318A (18V039). The new process requires photos after repairs are completed to verify proper installation, close the recall, and reimburse dealership claims by Mazda
’s warranty department.
May 17, 2023: Mazda
met with NHTSA to propose a voluntary campaign to understand conditions with Mazda
dealers in the field.
June 2, 2023 : NHTSA provided added guidance and confirmed support of a voluntary campaign .
June 5, 2023, to July 27, 2023 : This concern was cascaded through Mazda
’s internal process that reviews and approves voluntary campaign programs.
July 28, 2023 : Mazda
approves Special Service Program “SSPC9”.
July 29, 2023, to August 30, 2023 : Mazda
prepared for SSPC9 with a target of 249 vehicles for inspection to confirm positioning of the frontal air bag inflator. This included pulling applicable VINs and developing a list of vehicle owners most likely to respond to the voluntary campaign .
August 31, 2023, to November 23, 2023 : Through the new warranty claim process and not the planned voluntary campaign, four mis-installed cases were identified. Interviews were conducted and it was verified that technicians did not follow the revised repair instructions. In these four cases, Mazda
had dealerships tow vehicles back to be repaired properly.
November 23, 2023, to November 29, 2023 : Mazda
opted to cancel the field inspection program under SSPC9 and move toward proactive field action.
December 5, 2023 : Mazda
held a Quality Audit Committee meeting to review all available information to date. Mazda
decided to conduct a proactive field action on MY2004-2006 B-Series trucks that received previous recall remedy repairs.
December 21, 2023 : Mazda
has filed an amendment to adjust the Planned Owner Notification Begin and End Dates from January 12, 2024, to February 10, 2024.
To date, Mazda
has not received any field information reports.

Description of Remedy Program : Owners will be notified by mail and instructed to take their vehicle to an
authorized Mazda
dealer. Dealers will inspect the passenger side frontal air bag inflator orientation. If needed, the existing inflator will be re- installed in the correct position. There will be no charge for this service to vehicle owners. How Remedy Component Differs from Recalled Component : A reimbursement plan for repairs made in connection with this concern will not be offered, as vehicle owners have no awareness of this concern.
