Passenger Frontal Air Bag Inflator May Explode

Summary

MazdaeBay logo North American Operations (MazdaeBay logo) is recalling certain 2007-2012 CX-7, CX-9, and 2009-2012 Maza6 vehicles. These vehicles had their passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.

 

Remedy

MazdaeBay logo will notify owners, and dealers will replace the front passenger air bag inflator with an alternate one, free of charge. The recall began December 3, 2019. Owners may contact MazdaeBay logo customer service at 1-800-222-5500, option 4. MazdaeBay logo’s number for this recall is 1317F. Note: This recall supersedes recalls 16V356 and 17V012.

Passenger-Side Air Bag Inflator Replacement Procedure:

 

Felt Tape as needed for repairs as outlined in red in the repair instructions.

The Felt Tape will come in a plastic bag, (2) pcs of felt onto (1) liner.  Each Piece is 20mm x 50mm.  The center of the liner is kiss-cut so it bends so that each piece of felt can easily peel off.   The bag is 5โ€ x 7โ€.

Air Bag Module Removal:

WARNING:

  • Perform the replacement in a clean environmental. Oil, grease, water, etc. on components may cause the air bag modules to fail to deploy in an accident, which may cause serious injury. Do not allow oil, grease, water, etc., on components.
  • Inserting a screwdriver, etc., into the connector of an air bag module may damage the connec- tor and cause the air bag module to deploy improperly, which may cause serious injury. Do not insert any foreign objects into the connector.
  • Do not disassemble the vehicle, other than instructed here.
  • Do not make an attempt to directly supply electrical power to the module and the inflator.
  • Keep the module facing of the pad surface upwards at all times, except during replacement.
  • Do not use the module and new inflator if they were dropped on the ground.
  • Avoid module and inflator exposure to temperatures of 194 degrees F (90 degrees C) or more.
  1. Record the customerโ€™s preset radio stations and check the operation of the Passenger Airbag Deactivation (PAD) indicator in the information display to make sure it is operating correctly.

 

  1. Remove the passenger-side air bag module according to the instructions on MGSS.

CAUTION: When removing the dashboard, be careful when handling the information display. The Passenger Airbag Deactivation (PAD) indicator is very sensitive and can break easily.

 

  1. Lay a protective cloth on the work bench, then place the air bag module on it to avoid any damage or scratches to the surface.

 

Inflator Replacement:

NOTE:  To show the air bag module more clearly, the instrument panel trim was removed.

  1. Remove the M5 nuts from 4 locations.

 

  1. Remove the stopper plate and inflator.

 

  1. Attach the felt tape (A) to the retainer in 2 places.

 

a. Attach one piece of felt tape in the straight area of the retainer (A) using the red line (B) on the lower part of the straight edge as reference. Fold in half and paste inside the retainer.

 

CAUTION: DO NOT attach the felt tape past the red line (bolt side) to prevent it from contacting the assembly nut.

 

b. Attach the other felt tape on the other side of the retainer.

 

  1. Insert the new inflator into the retainer.

 

  1. Confirm that the inflator is assembled correctly.

 

NOTE:  Make sure to orientate the 1st and 2nd stage direction before inserting the inflator into the retainer.

  1. Attach the collar D to the 2nd stage side of the new inflator.

 

  1. Insert the new inflator and align the side of the retainer and guide line of the inflator (A).

NOTE:   Align the guide line (red line) within 5 mm from the retainer edge (B).

 

 

  1. Confirm that the inflator is assembled correctly.

NOTE: Pay attention to the direction of the connector. The connector direction is perpendicular to the bot- tom of the retainer.

 

  1. Install the new stopper to the retainer.

 

  1. Install the new M5 nuts in 4 locations.

 

Tightening torque: 34.5 ยฑ 2.6 in-lb (3.9 ยฑ 0.4 Nm)

 

Short-cord Replacement:

  1. Attach the SHORT-CORD-A/B to the vehicle with a tie wrap so that it does not contact any other surrounding parts.

 

  1. Install the new wire harness contained in the KIT (A) and fix the wire harness with a tie wraps (shop supply) at each point shown (B).

Air Bag Module Reinstallation:

  1. Install the passenger-side air bag module according to the instructions on MGSS.

  1. Reconnect the negative battery cable.

 

  1. Turn the ignition switch to the ON position.

 

  1. Verify that the air bag system warning light illuminates for approx. 6 sec., then goes out.

 

NOTE: If the air bag system warning light does not operate in the manner described above, there is a mal- function in the system. Inspect the system following the on-board diagnostic system procedure on MGSS (Restraints section).

 

  1. Re-enter the customerโ€™s preset radio stations.

 

  1. Proceed to โ€œC. CAMPAIGN LABEL INSTALLATIONโ€.

  1. Remove the connector of the harness assembly.

 

a. Pull the locking pin up using a thin tool at the tip.

 

b. Remove the clip from the coupler.

 

  1. Remove the M5 nuts from 4 locations.

 

  1. Remove the stopper plate and inflator.

 

  1. Attach the felt tape (A) to the retainer in 2 places.

 

a. Attach one piece of felt tape in the straight area of the retainer (A) using the red line (B) on the lower part of the straight edge as reference. Fold in half and paste inside the retainer.

 

CAUTION: DO NOT attach the felt tape past the red line (bolt side) to prevent it from contacting the assembly nut.

 

  1. Attach the other felt tape on the other side of the retainer.

 

NOTE:   Paste along the side of the bracket.

 

  1. Insert the new inflator into the retainer

 

NOTE:  Make sure to orientate the 1st and 2nd stage direction before inserting it into the retainer.

a. Attach the collar D to the 2nd stage side of the new inflator.

Tightening torque: 34.5 ยฑ 2.6 in-lb (3.9 ยฑ 0.4 Nm)

 

  1. Connect the replacement wire harness at 2 locations.

 

a. Make sure the connectors are fitted properly.

 

b. The connectors direction is perpendicular to the bottom of the retainer.

 

Air Bag Module Reinstallation:

  1. Install the passenger-side air bag module according to the instructions on MGSS.

  1. Reconnect the negative battery cable.

 

  1. Turn the ignition switch to the ON position.

NOTE:   To show the air bag module more clearly, the instrument panel trim was removed.

 

  1. To prevent the inflator from exploding by mistake during its removal, short-circuit the harnesses.

NOTE:   The harness will not be reused after shorting it.

 

a. Cut off the PVC tube to expose the wires.

 

NOTE:  Cut off the corrugated tubing for the two wire harnesses indicated by the arrows.

 

b. Strip the insulation from the two wires as shown.

 

c. Connect the two wires by twisting them together (for both wire harnesses).

 

d. Cut the two wires midway (A) between the connector and the splice (B) as shown.

 

CAUTION: Make sure to cut the wires on either side of the connector between the connector and the splice.

 

  1. Remove the five (5) nuts (A).

 

NOTE:  DO NOT excessively depress as the module body may detach from the air bag module.

 

  1. Remove the stopper plate (A).

 

NOTE:  Discard four (4) of the nuts and the stopper plate. Keep one of the nuts.

 

  1. Push the inflator end with fingers, then remove it by sliding it in the direction shown.

 

  1. Attach the felt tape (A) to the retainer in 2 places.

 

a. Attach one piece of felt tape in the straight area of the retainer (A) using the red line (B) on the lower part of the straight edge as reference. Fold in half and paste inside the retainer.

 

CAUTION: DO NOT attach the felt tape past the red line (bolt side) to prevent it from contacting the assembly nut.

 

b. Attach the other felt tape on the other side of the retainer.

 

  1. Insert the new inflator into the retainer

 

NOTE:  Make sure to orientate the 1st and 2nd stage direction before inserting it into the retainer.

  1. Install the new stopper to the retainer.

 

  1. Install the new M5 nuts and one used nut (in position 3) as shown, then tighten the 5 nuts to the specified torque in the order shown.

 

Tightening torque: 34.5 ยฑ 2.6 in-lb (3.9 ยฑ 0.4 Nm)

 

CAUTION:  DO NOT over tighten the nuts or the stud bolts could be removed from their press-fit points.

 

  1. Connect the replacement wire harness at 2 locations.

 

NOTE:  Make sure the connectors are fitted properly.

 

Air Bag Module Reinstallation:

  1. Install the passenger-side air bag module according to the instructions on MGSS.

  1. Reconnect the negative battery cable.

CONDITION OF CONCERN

In the subject vehicles, continued exposure to high levels of absolute humidity may cause the passenger frontal air bag inflator housing to rupture and deploy abnormally in the event of a crash necessitating deployment of the passenger frontal air bag. An inflator rupture could result in metal fragments striking the front passenger or other vehicle occupants, resulting in serious injury or death.

Dealers are to replace the passenger side frontal air bag inflator of subject vehicles with a permanent remedy one.

 

MANDATORY AIR BAG INFLATOR RETURN

The original air bag inflator(s) should be returned by your dealership every 30 days or once 200 inflators have been accumulated. All inflator part numbers can be counted and shipped together for these returns. To schedule a return, please contact XPO at (877) 650-3476. Detailed air bag inflator return instructions can be found on MGSS and the Dealer Assistance Group websites. Failure to return the original inflator will result in a Warranty Claim Debit.

Subject: Notification of Takata Safety Recall Expansion to the DIR5 group of vehicles to existing Safety Recalls 1317F and 2618F.

Recall 1317F expansion: DIR5

MazdaeBay logo Motor Corporation has decided that a defect which relates to motor vehicle safety exists in vehicles equipped with certain type of passenger frontal air bag inflator, and to conduct a recall campaign for certain 2009-2012 Mazda6, 2007-2012 CX-7 and 2007-2012 CX-9 vehicles. There are 70,563 U.S. and U.S. Territory vehicles involved in this recall. The repair is identical to the current repair for vehicles in the existing recall. This recall is an expansion of Safety Recall 1317F

– Passenger Air Bag Inflator Replacement filed with NHTSA on October 12, 2018.

 

Recall 2618F expansion: DIR5

MazdaeBay logo Motor Corporation has also decided that a defect which relates to motor vehicle safety exists in vehicles equipped with certain type of passenger frontal air bag inflator, and to conduct a recall campaign for 2003-2008 Mazda6, 2006-2007 Mazdaspeed6, and 2004-2005 MPV, and 2004-2005 RX-8 vehicles. There are 47,613 U.S. and U.S. Territory vehicles involved in this recall. The repair is identical to the current repair for vehicles in the existing recall. This recall is an expansion of Safety Recall 2618F – Passenger Air Bag Inflator Replacement filed with NH TSA on June 15, 2018.

For both recalls, all affected vehicles have been repaired with “like-for-like” inflators from prior Takata recalls. These “like-for-like” inflators were an interim remedy until a sufficient amount of permanent parts were available. These affected vehicles will now need to be repaired once more with the permanent remedy inflator.

In the subject vehicles, activation of a non-desiccated ammonium nitrate inflator with degraded propellant may result in an inflator rupture. An inflator rupture may cause metal fragments to pass through the air bag and into the vehicle interior at high speed, which may result in injury or death to vehicle occupants.

Dealers are to replace the passenger frontal air bag inflator of the affected subject vehicles and the affected vehicles will display Not Launched in eMDCS today, November 4th , 2019. As parts are available for all vehicles in DIR5, dealers can perform the repair if a customer requests it even in Not Launched status.

Customers affected by this recall are scheduled to be notified by first class mail no later than December 16, 2019 and will be asked to make an appointment with a MazdaeBay logo dealer to have the passenger frontal air bag inflator replaced with a permanent remedy part for recall 1317F and 2618F.

 

Important Safety Notice:

The National Traffic and Motor Vehicle Safety Act prohibits the delivery of any subject vehicle without performing the necessary repair for defects or failures. Failure to perform applicable recalls before delivery can result in extensive dealer fines and penalties from the Federal Government. Therefore, you must complete this recall for all affected vehicles in your inventory prior to delivery.

To help you effectively perform this recall, MazdaeBay logo has developed the following resources:

  1. Repair Procedures and Parts and Warranty information will post on MGSS (MazdaeBay logo Global Service Support) websites via MXConnect as soon as possible, hopefully by November 7, 2019.

Passenger side frontal air bag may explode on MY 2009-2012 Mazda6,  MY2007-2012 CX-7 and MY2007-2012 CX-9.

On May 3, 2016 : As a result of testing and analysis conducted by Takata and independent research organizations, NHTSA concluded that non-desiccated Takata PSAN (phase-stabilized ammonium nitrate) inflator would reach threshold level of degradation, causing the inflator to become dangerous. Therefore, NHTSA ordered Takata to file phased DIRs, in accordance with the schedule set forth in the amendment to the Consent Order.

 

From May 16, 2016 to October 16, 2018; MazdaeBay logo has submitted relevant recalls in phased manner, as DIR#1 through DIR#4, in accordance with three DIRs Takata submitted to NHTSA.

 

In the middle of October, 2019: The replacement parts using non-PSAN based inflators have become available for all subject vehicles which have ever been replaced with non-desiccated PSAN inflators as an interim remedy to avoid an immediate safety risk posing air bag rupture, in the past recall, 16V356 and 17V012.

 

On October 28, 2019; MazdaeBay logo held the Quality Audit Committee and decided to conduct a recall covering the subject vehicles replaced with like-for-like parts, MY2009-2012 Mazda6, MY2007-2012 CX-7 and MY2007-2012 CX-9.

 

Updated July 27 2020 โ€“ This amendment is to inform of MazdaeBay logoโ€™s intent to implement the โ€œotherโ€ reporting category for removed vehicles in Quarterly Reporting as permitted in the Third Amendment to the Coordinated Remedy Order (โ€œACROโ€), Paragraphs 45 through 49.  As a condition of the ACRO, all affected vehicles will remain active, or โ€œliveโ€, across MazdaeBay logoโ€™s data systems such that any search of โ€œotherโ€ removed VINs will return an open recall status.  For this reason, the Number of potentially involved vehicles in this report will remain the same.  However, for reference, initial counts of vehicles in the โ€œotherโ€ reporting category are indicated in attached supplement.  Note that these counts are current as of the second calendar quarter 2020.  Future changes to overall Total Removed counts, including โ€œOther,โ€ will be reflected in quarterly reporting as well as monthly dashboard reporting until otherwise directed by the agency.

Air Bag Inflator Rupture

Dated opened: February 24, 2015

The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers. All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.

During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers. Nonetheless, at ODI’s insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation. The regions recalled initially included Florida, Puerto Rico, Hawaii, and the U.S. Virgin Islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures. As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.

Takata’s initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida. Accordingly ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas. Takata’s testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions. During PE14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region.

Also during PE14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina. Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls. Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity. Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing. All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas.

The investigation now includes all manufacturers and vehicles known to be affected at this time. ODI’s investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy.
