Mazda
North American Operations (Mazda
) is recalling certain 2004-2007 Mazda3 vehicles. During air bag deployment, the plastic emblem on the steering wheel air bag module cover may shatter, and project plastic fragments into the vehicle.
Remedy
Dealers will replace the air bag module cover, free of charge. Owner letters were mailed between August 28, 2021 and October 28, 2021. Owners may contact Mazda
customer service at 1-800-222-5500 Option 4. Mazda
’s number for this recall is 4921G.

Chronology of Defect
Steering wheel logo may shatter during air bag deployment on MY 2004 through MY2007 Mazda3
September, 2015: Mazda
received first field report of “The occupant injured face by scattered fragments of the logo when the air bag is activated in a crash” from outside the US market.
December, 2015 through February, 2016: The part supplier and Mazda
started to investigate the root cause. Hydrolysis was confirmed to occur on the broken logo which was made of polyurethane. The hydrolysis was hypothesized to accelerate deterioration of the logo and reduce its strength.
February, 2016: As a result of high heat and humidity durability test, the molar weight of the logo was found to be reduced, which indicated the hydrolysis was duplicated in the polyurethane sample.
March, 2016: It was found that the supplier made a change to the material of the logo, from polyurethane to polyester in June, 2006. This material change was intended to improve the logo appearance. As a result of later investigation, the hydrolysis was confirmed to not occur on logo made of polyester. This material change was also found to incidentally be a corrective measure for subsequently produced vehicles.
April, 2016: Mazda
found that the logos for other vehicle models were not made of polyurethane, which may be subject to hydrolysis. Accordingly, there is no possibility for this defect to occur on other vehicle models.
January, 2017: Mazda
determined at this time that the projected fragments could only cause superficial damage or injury and the safety risk was low because the plastic fragments are very light in mass. Up to this time, considering the occurrence rate of collisions necessitating air bag deployments in the field, Mazda
had estimated the occurrence rate of this defect in the field would be low.
February, 2017: Mazda
judged that the degree of injury caused by this concern was equivalent to the type of injury that can occur during a normal air bag deployment in a crash. Accordingly, Mazda
decided to monitor any field occurrences.
June, 2018; Mazda
received second field report of “The occupant injured the arm by scattered fragments of the logo when the air bag was activated in a crash” from outside the US market.
August, 2018: Mazda
received third field report of “The occupant injured the face by scattered fragments of the logo when the air bag was activated in a crash” from outside the US market.
December, 2018: Mazda
received fourth field report of “The occupant injured the face by scattered fragments of the logo when the air bag was activated in a crash” from outside the US market.
February, 2019: Mazda
and the parts supplier investigated the second to fourth returned parts. Based on the investigation result the root cause was similar to that of the first reported field incident which occurred in September, 2015. Mazda
judged that the occurrence rate was low.
March, 2019: Mazda
received the fifth field report, which reported “The occupant injured left eye by projectile fragments of the logo when the air bag was activated” from outside the US market.
May, 2019: Mazda
attempted to collect parts to examine the material condition of the logo, but no parts could be recovered for analysis. Mazda
decided to continue monitoring any occurrences in the field in an attempt to gather further information.
July, 2019: Mazda
received sixth field report of “The occupant injured eye by projectile fragments of the logo when the air bag was activated” from outside the US market. Mazda
subsequently received a seventh field report of “The occupant injured face by projectile fragments of the logo when the air bag was activated” from outside the US market. At this time U.S. lawsuits alleging injury possibly caused by this defect were received.
August, 2019: Mazda
received tenth field report of “The occupant injured face by projectile fragments of the logo when the air bag was activated” from outside the US market.
September, 2019; As Mazda
received the fifth through tenth field concerns and law suits, Mazda
decided to continue monitoring occurrences in the field because sufficient information and part analysis was still unavailable.
November, 2019: As a precaution in case it was judged that proactive field actions may be necessary in foreign markets, Mazda
began to prepare the air bag module cover as a replacement part.
June, 2021: Mazda
has received no field reports since the tenth field incident globally. However, considering the nature of the defect’s root cause, Mazda
was concerned about a possible increase in injury cases caused by this defect in the future. Accordingly, Mazda
has decided to judge whether to take proactive field actions.
June 25, 2021: Eight field incidents have been reported as a result of this defect outside the US, and two US lawsuits were received alleging injury. Mazda
held a Quality Audit Committee meeting to review all available information to date. Out of an abundance of caution, Mazda
has determined to conduct a proactive field action on certain MY2004 through MY2007 Mazda3 vehicles in the US and US Territories.
August 27, 2021: Mazda
met with NHTSA-RMD on August 3 2021 to explain a change in the recall schedule due to an unexpected change in parts availability. As advised by the agency, Mazda
is submitting this amended report to revise the recall schedule to include a phased parts available launch in the U.S. and Puerto Rico.

What is the problem?
The plastic logo on the steering wheel air bag module cover may shatter and project fragments
into the vehicle interior during an air bag deployment in a crash. Due to an improper material
specification, the plastic logo may become brittle. In this condition, the logo may shatter during
a normal air bag deployment. Plastic fragments of the shattered logo may hit occupants,
increasing the possibility of serious injury to occupants. There is no prior warning for this defect.

Use latex or vinyl, powder-free gloves during this repair.
Do not use any power tools to remove the steering wheel air bag cover.
Use the smallest pair of side cutting pliers necessary to carefully cut the
plastic of the cover.
The air bag material will tear easily. If you tear or rip the air bag
material, please order and replace the air bag module (cannot be
claimed under 4921 G) and notify Dealer Recall Help before the vehicle
leaves the dealership.

2. Remove the Steering Wheel Air Bag Cover: DO NOT USE POWER TOOLS TO REMOVE BOLTS.
Using any type of power tool may shear the inflator stud and require replacement of the airbag
module assembly. This is not covered under this recall and will be the dealer’s responsibility.

PLASTIC BETWEEN THE AIR BAG MODULE METAL FRAME HOUSING AND THE STEERING WHEEL
COVER. DO NOT CUT OR PRY AT THE PINK COLOR AIR BAG MATERIAL. GOING TOO DEEP OR
USING THE INCORRECT TOOL WILL LIKELY CUT OR SCRATCH THE AIR BAG MATERIAL. IF THIS

5. Remove the Air Bag Module from the Driver Steering Wheel Cover

A. The Air Bag Module frame attaches to the cover with 2 snap clips/tabs on each side and 3
each on the bottom and top ( 10 clips/tabs total). See photos.

slot to allow for easy removal of each clip between the air bag module frame and steering

6. Once all clips are loose, gently pull out the air bag module steadily in a straight up motion. You may
need pull up the back, then the front or the other way around. NOTE: lnflator is shown in this picture.

7. Once removed, place the air bag module on a scratch free surface as shown below and visually
check the ,iir bag, maLerial for any cu Ls or Lears. I he Lop of Lhe air bag, will have a “X” pauern and
this is normdl. lfthere are any cuts or tears, the airbag must be replaced. If torn please contact
Dealer Recall Help with a photo of the tear and what caused the tear/rip. Also inspect the 10
tabs to ensure they are not deformed.

The 4 locations of stay tabs needs to be located above the steering cover edge. Note there are
4 tabs.

Installation of Inflator
13. Remove the two used nuts that were temporarily used to fasten airbag and housing. Discard all
old/used nuts as they must not be reused.
14. Install the inflator using new nuts by finger tight first, then tight with the specified torque in the
order of ①-②-③-④ as shown in figure. Tightening nut torque: 2.9 – 5.9 N・m, 26 (in-lbs) – 52
(in-lbs). This is 2-4 ft-lbs!
CAUTION: Do not overtighten the nuts. Make sure to set the correct tightening torque.
Final inspection: Confirm that all 10 tabs are properly installed, the 4 stay-tabs are on the cover edge
and all 4 nuts are new and correctly mounted. Accordingly, mark each point, as checked and found
to be OK, using a white pen or equivalent.

This notice is sent to you in accordance with the National Traffic and Motor Vehicle Safety Act.
Mazda
Motor Corporation has decided that a defect which relates to motor vehicle safety
exists in certain 2004-2007 model year Mazda3 vehicles produced from June 24, 2003 through
June 22, 2006. If you received this notice, your vehicle is included in this Recall.
What is the problem?
The plastic logo on the steering wheel air bag module cover may shatter and project fragments
into the vehicle interior during an air bag deployment in a crash. Due to an improper material
specification, the plastic logo may become brittle. In this condition, the logo may shatter during
a normal air bag deployment. Plastic fragments of the shattered logo may hit occupants,
increasing the possibility of serious injury to occupants. There is no prior warning for this defect.
What will Mazda
do?

dealer will replace the air
bag module cover with an improved part.
The repair will be performed at no cost to
you. The air bag inflator will not require
replacement. There will be no charge for
this service.
How long will it take?

It will take approximately one hour to complete the replacement of the air bag module cover;
however, your Mazda
dealer may need your vehicle for a longer period of time. Mazda
will
provide alternate transportation, if needed, when your vehicle is at an authorized Mazda![]()
dealership for a recall repair. Please schedule an appointment with an authorized Mazda![]()
dealer so they can discuss and accommodate your needs.

Owner Notification:
Vehicles will display in eMDCS as “Preliminary Notification” tomorrow, August 27, 2021 as a sufficient level of
parts remain in transit to support a launch. The oldest vehicles in high humidity areas will be opened as parts
become available by the end of September, and all vehicles will be in open status by the end of October
2021. Updated communications will be sent as sufficient part levels become available.
If a customer insists on repairing their vehicle, please contact Dealer Recall Help on OneMazda to arrange a
