Summary
Mazda North American Operations (Mazda
) is recalling certain 2020-2021 Mazda2 vehicles. The headlights are missing adjustment prevention caps, which can allow the headlights to be improperly adjusted. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 108, “Lamps, Reflective Devices, and Associated Equipment.”
Remedy
Dealers will install the missing adjustment prevention caps, free of charge. Owner notification letters were mailed December 28, 2022. Owners in Puerto Rico may contact Mazda customer service at (787) 620-7546. Owners in the United States may contact Mazda
customer service at 1-800-222-5500 Option 6. Mazda

On September 7, 2022: Mazda detected headlamps with horizontal aiming prevention caps missing from part supplier. Mazda
began to investigate.
On the September 12, 2022: Mazda identified the root cause and conducted a mass production change.
Middle of November 2022: Mazda investigated potentially affected models and population with this defect.
November 24, 2022; Mazda held a Quality Audit Committee meeting to review all available information to date and decided to conduct a recall covering the subject vehicles in the U.S. and U.S. territories. No accidents, injuries or deaths have been reported because of this defect.

Mazda began to investigate. On the September 12, 2022: Mazda identified the root cause and conducted a mass production change. Middle of November 2022: Mazda investigated potentially affected models and population with this defect. November 24, 2022; Mazda held a Quality Audit Committee meeting to review all available information to date and decided to conduct a recall covering the subject vehicles in the U.S. and U.S. territories. No accidents, injuries or deaths have been reported because of this defect. Description of Remedy : Description of Remedy Program : Owners will be notified by mail and instructed to take their vehicle to a Mazda dealer. Dealers will install a headlamp horizontal aim adjustment prevention cap on each headlamp. There will be no charge for this service to vehicle owners. As subject models are covered under new vehicle warranty, a reimbursement program will not be offered. Mazda Motor Corporation has decided that certain 2020-2021 Mazda2 vehicles, produced from November 1, 2019 through December 9, 2021, fail to conform to Federal Motor Vehicle Safety Standard No. 108, “Lamps, Reflective Devices and Associated Equipment.” If you received this notice, your vehicle is included in this recall. What is the problem? On the subject Mazda2 vehicles, the headlight are missing adjustment prevention caps, which may allow the headlights to be improperly adjusted. Improperly adjusted headlights can reduce visibility, increasing the risk of a crash. What will Mazda do? Your Mazda dealer will install the missing headlight adjustment prevention caps. The repair will be performed at no cost to you. How long will it take? It will take approximately 30 minutes to complete the repair; however, your Mazda dealer may need your vehicle for a longer period of time. Mazda will provide alternate transportation, if needed, when your vehicle is at an authorized Mazda dealership for a recall repair.
