Daytime Running Lights May Fail

If the DRL fails, the vehicle’s visibility may be reduced to other drivers on the road, increasing the risk of a crash.

Summary

Mazda North American Operations (Mazda) is recalling certain 2016 CX-5 vehicles. The LED Daytime Running Lights (DRL) may fail while driving.

 

Remedy

Mazda will notify owners, and dealers will inspect the headlights, and replace the sealing gasket or headlight assemblies, as necessary, free of charge. The recall began March 20, 2020. Owners may contact Mazda customer service at 1-800-222-5500 Option 4. Mazda’s number for this recall is 4320A.

1. VEHICLE INSPECTION PROCEDURE

Verify that the vehicle is within the following ranges:

AFS: Adaptive Front Lighting system which enhances the range of visibility when the headlights are turned on by pointing the optical axis of the headlights in the direction in which the vehicle is advancing according to the steering operation. The swivel actuator constantly detects the headlight swivel angle from the hall sensor built into the actuator, and inputs the swivel angle to the AFS control module via LIN communication.

A. VISUAL INSPECTION

  1. Park the vehicle and turn on the engine.
  2. Turn on the low beam.

Check the illumination of the DRL position lighting and determine if they are lighting up and not flickering. (Marked by yellow area with shown the image) For more detail, refer to inspection chart below.

 

OK - DRL is illuminated.

Judgement

  • If both Daytime Running Lamp (DRL) does not light up or the DRL is flickering, replace the both headlight assemblies and complete the repair.
  • If either Daytime Running Lamp (DRL) does not light up or the DRL is flickering, replace the affected headlight assembly and go to the next step, B. (Inspect working headlight production date)
  • If both Daytime Running Lamp (DRL) light up, and are not flickering, go to next step B. (Inspect working headlight production date)

 

NOTE: Ensure that you have accurately determined the production date of the lamp and the status of the proper DRL operation. Lamps are being collected and tested for failure analysis.

 

B. Head Light Production Date Inspection

  1. Open the hood.
  2. Check the production date by confirming information sticker.

Judgement

  • If both headlamp assemblies are not in the affected production date, the repair is complete. Submit for inspection to close the recall.
  • If one or both of the headlamp assemblies are in the affected production date, replacement of gaskets and/or installation of gas absorption sheets is necessary. Go to step C.
  • If the information sticker is missing or production date cannot be confirmed visually due to dirty/burned surface, replacement of gaskets and/or installation of gas absorption sheets is necessary. Go to step C.

 

C. FRONT BUMPER & COMBINATION LIGHT REMOVAL/INSTALLATION

Caution

  • Affix the protective tape to the position (vehicle body side) shown in the figure to prevent scratches and damage.

  2. Remove fasteners A.

  1. Pull the front over fender in the direction of the arrow shown in the figure, set the front over fender out of the way while detaching clips B.

 

Caution

  • After removing clips B, insert a rag between the front fender panel and the front over fender to prevent the front fender panel and clips B from being damaged.

  2. Pull the front bumper in the direction of the arrow (1) shown in the figure, then remove tabs

 

Caution

  • The front bumper and front bumper slider are engaged firmly. If they are disengaged forcibly the bumper could fall and be damaged. Perform the servicing carefully when disengaging the front bumper from the front bumper slider.
  • When disengaging the front bumper from the front bumper slider, the front bumper could fall and be damaged. Support the front bumper so that it does not fall.

  1. Remove the front bumper from the front bumper slider.
  2. Pull the front bumper in the direction of the arrow (2) shown in the figure and remove it while detaching guides G.

  1. Disconnect the front fog light connector. (with front fog lights)
  2. Disconnect the front ultrasonic sensor connector. (with parking sensor system)

 

Caution

  • After removing front bumper, it may hit the front over fender and cause a damage and/or injury. Perform the following procedure to prevent the front over fender from being damaged. — Fix the front over fender and front fender panel with protective tape.

FRONT COMBINATION LIGHT REMOVAL/INSTALLATION

Warning

  • Incorrect servicing of the discharge headlights could result in electrical shock. Before servicing the discharge headlights, always refer to the service warnings. (See DISCHARGE HEADLIGHT SERVICE WARNINGS.)

 

Note

  • Fogging or condensation on the inside of the front combination lights may occur due to a natural phenomenon occurring as a result of a temperature difference between the interior and exterior of the combination lights. However, it has no effect on the light performance because the temperature inside the front combination lights rises after illuminating the headlights or a period of time has elapsed.

 

  1. Disconnect the connector.

  1. While pressing the clip tab in the direction of the arrows (1) shown in the figure, press the clip in the direction of the arrow (2) shown in the figure to detach the clip tab and front combination light (LH of discharge type).
  2. Remove the clip (LH of discharge type).

  1. To prevent scratches or damage, affix protective tape to the position shown in the figure.

 

Caution

  • When the front combination light is removed from the body, perform the procedure after affixing protective tape to the body. Otherwise, the body could interfere with the front combination light and cause scratching or damage to the body.

  1. Pull the front combination light in the direction of the arrow shown in the figure and pull out the front combination light pin from the body.

Caution: Be careful not to damage the headlight lens (scratch etc…) or deform the terminal.

In order to easily reference locking/unlocking the connector, ①Make sure it is fully locked position (fully turned left and stopped position) then ②Mark before removing.

  1. Replace O ring for connector A
  • Thread a piece of thin spare wire or string tie through the locking tab of the connector

* This is to prevent the headlamp connecter from falling into the headlamp assembly (Connector B)

  • Unlock the connector from the headlamp and make room between housing and connector to allow for the removal of the O ring.

 

Note:

Recommend watching video B and replacing that O ring first along with installing the gas absorption sheet. It is the easier of the two O rings.  Then watch video A and replace connector A O ring. It will be more difficult as the connector cannot be turned 90 degrees as shown in the connector B photos below.

 

  • Using a pick tool, hook and pull the gasket off of the connector and out of the headlamp.

  1. Replace the gaskets with the new black supplied gaskets of connector A and connector B and install the two gas absorption sheets.

 

Note:  You may find a black gasket from a prior repair at connector B. Replace it anyway. Install gas absorption sheet at connector B.   Go to gas absorption sheet installation procedure.

  1. Install the gasket with new one (black one). Thread the new gasket onto the spare wire or string.

–  Push the O ring gasket into the four hooks at the bottom of the connector.

Note:  Connector A will not drop down far enough to turn 90 degrees. This O ring has limited room and visibility. Watch Video A

  1. Pull the wire and remove from connector. Lock the connector shown the image below.

  1. Unlock and make a gap between housing and connector.
  1. Use a pick tool, pull the gasket toward the outside.

  1. Prepare a gas absorption sheet and use your finger, or tweezer, short ruler to install.
  2. Peel off the adhesive tape back of gas absorption sheet.

  1. Install gas absorption sheet (2 pieces per one side) inside the connector hole.

  2. Make sure all connector is locked. (There is marked where the lock position at yellow circle.)

  3. Repair other headlamp if necessary. Go back to Step D
  4. Install the head light(s) with front bumper.
  5. Check the headlight aiming.

F. CAMPAIGN LABEL INSTALLATION 

  1. Fill out a blue “Campaign Label” (9999-95-065A-06) with Campaign No: “4320A”, your dealer code, today’s date.

CONDITION OF CONCERN

The LED type daytime running lights (DRL) may flicker or fail to illuminate while driving.

If one or both DRLs fail to illuminate, the vehicle’s visibility may be reduced to other drivers on the road, increasing the risk of a crash.

 

Causal factor:

Due to an inappropriate material specification of the sealing gasket attached to the LED daytime running light (DRL) wiring connector, the gasket can release sulfur which may corrode the LED circuit controlling the DRL. If corrosion of the circuit develops, the circuit may lose continuity, causing flickering and/or failure of DRL illumination. One or both front DRLs on affected vehicles could be affected by this defect.

 

OUTLINE OF REPAIR

Inspect the DRL headlights, and if needed, replace the sealing gaskets with improved ones and attach desulfurizing materials (gas adsorption sheets). If the DRL headlight(s) is/are exhibiting affected condition (flicker/fail to illuminate), replace the head lamp unit(s).

Dear Mazda Owner:

 

What is the reason for this notice?

Mazda previously sent a notice in May 2020 which advised that parts supplies under recall 4320A had been impacted due to COVID-19 for vehicles requiring replacement of headlamp assemblies. Because of this issue, complete headlamp assemblies to support this recall repair were not available for several months. If you are a recipient of this letter, please read the information below as headlamp assemblies are now available.

 

What is the problem?

The LED type daytime running lights (DRL) may flicker or fail to illuminate while driving which may reduce vehicle visibility to other drivers on the road, increasing the risk of a crash. One  or both front DRLs on affected vehicles could be affected by this defect. As of this mailing your vehicle is unrepaired.

 

What should you do?

Mazda has made available all repair parts needed for vehicles registered in the following states: Delaware, Hawaii, Illinois, Louisiana, Maine, Massachusetts, Mississippi, Missouri, New Hampshire,  New Jersey,  New York, North Carolina, Pennsylvania, Rhode Island, Texas, Utah, Vermont, Virginia, and West Virginia and Washington, DC.  Please contact any authorized Mazda dealer to schedule an appointment to have your CX- 5 vehicle  inspected  or repaired for free as soon as possible.  You must visit a Mazda  dealer  to  inspect  or repair  your vehicle  whether  or not  the headlamps  have been replaced prior  to the recall launch  in March 2020.

 

If you have visited a Mazda dealer recently and had your  vehicle  repaired under  recall 4320A, please disregard this notice. However, if your DRLs flicker or fail to illuminate after the recall repair, please contact your nearest Mazda dealership.

 

How long will it take?

The time required for the repair will depend on the inspection. If your vehicle is not equipped with the affected headlight units and only the inspection is necessary, approximately half an hour will be required to complete the inspection. If repairs are necessary on your vehicle, it will take approximately two hours to complete; however, your Mazda dealer may need your vehicle for a longer period of time.

Subject: Safety Recall 4320A 2016MY Mazda CX-5 Daytime Running Lights May Fail LIMITED RELAUNCH PLEASE READ

 

As communicated in May, due to the Covid-19 pandemic, Mazda experienced a severe supply shortage of the headlamp assembly units associated with this recall. As a result, repairs needing a headlamp assembly replacement were suspended while the supply constraints was resolved, and sufficient supply became available to resume repairs. We are pleased to report the supply flow has recovered to a level where headlamp assembly replacement can resume beginning in two phases.

 

Phase 1: Beginning the week of December 14th , Mazda will relaunch owner letters to customers with unrepaired vehicles in the following states: Delaware, Hawaii, Illinois, Louisiana, Maine, Massachusetts, Mississippi, Missouri, New Hampshire, New Jersey, New York, North Carolina, Pennsylvania, Rhode Island, Texas, Utah, Vermont, Virginia, and West Virginia and Washington, DC.

 

Customers will be advised to contact their local Mazda dealership to schedule an appointment and have their CX-5 vehicle inspected/repaired.

 

Phase 2: In early 2021, Mazda will open the remaining states not included in Phase 1 and notify the remaining customers by US Mail to contact a Mazda dealer for inspection/repair.

 

Dealer Action Required

  • If a phase 1 vehicle requires one or both headlamps due to DRL failure, please replace and submit the claim to close the recall. In all other states, if neither DRL or only one DRL illuminates, parts are not available to perform the recall. Please inform the customer that both the low beam and high beam lamps will function normally, and customers can utilize their low beam lamps to provide additional daytime visibility.

 

  • Note: Exceptions will be made on a case by case basis for the following situations:
    • US Military moving into/out of the USA or those that cannot get onto a military base
    • Customers who have a written fix-it ticket from local or state police
    • Vehicles being prepped for CPO sale at a Mazda dealer
    • In the above cases, or if there are any other extenuating circumstances, please contact Dealer Recall Help on MXConnect > Warranty with the required information and the recall team will assist with parts ordering. 
  • Body shop or customer pay orders will be Contact the Dealer Assistance Group via email if you need to order headlamp assemblies for Body Shop or Customer Pay situations not related to Recall 4320A.

 

In May Mazda transitioned all existing stock backorders offline until the PDC supply levels could support headlamp repairs. In anticipation to relaunch headlamp  repairs, these stock backorders from May will begin filling on Friday, December 11 th. These sales orders will fill according to the appropriate phase for your dealership. Orders will start arriving on your next DDS delivery day and it could take a few days for all pieces to arrive. In eMDCS you can reference the purchase order number for these orders as “R4320A PUSH”.

Mazda previously sent a notice in May 2020 which advised that parts supplies under recall 4320A had been impacted due to COVID-19 for vehicles requiring replacement of headlamp assemblies. Because of this issue, complete headlamp assemblies to support this recall repair were not available for several months. If you are a recipient of this letter, please read the information below as headlamp assemblies are now available.

 

What is the problem? 

The LED type daytime running lights (DRL) may flicker or fail to illuminate while driving which may reduce vehicle visibility to other drivers on the road, increasing the risk of a crash. One  or both front DRLs on affected vehicles could be affected by this defect.

 

What should you do?

As of this mailing your vehicle is unrepaired. All parts to repair vehicles under this recall are now available. Please contact any authorized Mazda dealer to schedule an appointment to have your CX-5 vehicle inspected or repaired for free as soon as possible. You must visit a Mazda dealer to inspect or repair your vehicle whether or not the headlamps have been replaced prior to the recall launch in March 2020.

 

If you have visited a Mazda dealer recently and had your vehicle repaired under recall 4320A, please disregard this notice. However, if your DRLs flicker or fail to illuminate after the recall repair, please contact your nearest Mazda dealership.

 

How long will it take?

The time required for the repair will depend on the inspection. If your vehicle is not equipped with the affected headlight units and only the inspection is necessary, approximately half an hour will be required to complete the inspection. Concern Outline:

The LED Daytime Running Lights (DRL) on certain subject vehicles may unexpectedly flicker or fail to illuminate while driving. If one or both DRLs fail to illuminate, surrounding traffic and pedestrians may not be able to see the affected vehicle, especially when the headlights are off in low light conditions.

Due to an inappropriate material specification of the sealing gasket attached to the LED daytime running light (DRL) wiring connector, the gasket can release sulfur which may corrode the LED circuit controlling the DRL. If corrosion of the circuit develops, the circuit may lose continuity, causing flickering and/or failure of DRL illumination. One or both front DRL’s on affected vehicles could be affected by this defect.

 

For all subject vehicles:

Inspect the headlights and, if needed, replace each with an improved sealing gasket and attach desulfurizing materials (gas adsorption sheets) inside the headlight housing or if the headlights are inoperable, replace the headlight unit(s).

As a reminder, all vehicles under this recall will show as “Not Launched” status in eMDCS on February 1 0, 2020. For customers that inquire about this recall and demand a repair priorto launch, dealers can elect to perform the repair once repair procedures are available on the Mazda Global Support System (MGSS).

 

Owner Notification:

Mazda will notify owners of affected vehicles by first class mail no later than April 3, 2020. Owners will be asked to bring their vehicle to the nearest Mazda dealership for repairs.

On February 10, 2020, the Mazda Warranty Department announced this Safety Recall Campaign for certain 2016 Mazda CX-5 vehicles. Parts & Warranty Information and Repair Procedures will be uploaded to the Mazda Global Support System (MGSS) on February 25, 2020.

 

Action Required:

Please make sure to follow the steps outlined in the Repair Procedure. Once both headlamps are inspected, determine what repair action is required. Some headlight assemblies will be inspection only, some headlight assemblies will require replacement of 2 improved sealing gaskets and desulfurizing materials (gas adsorption sheets) inside each headlight housing, or if the DRL portion of the headlights are inoperable, replacement of the headlight assembly.

 

To support repairs for functioning DRL headlight assemblies, a small push shipment was uploaded yesterday, February 24. These orders should arrive on your next DDS delivery day and in eMDCS you can reference the purchase order number for these orders as “R4320A PUSH”.

 

As a reminder, all vehicles under this recall will show as “Not Launched” status in eMDCS until the Owner Letters are mailed out. For customers that inquire about this recall and demand a repair prior to launch, dealers can elect to perform the repair and then submit a claim through eMDCS. Mazda will notify owners of affected vehicles by first class mail no later than April 3, 2020. Owners will be asked to bring their vehicle to the nearest Mazda dealership for repairs.

On the affected headlight unit(s), desulfurizing materials will be installed and the sealing gaskets will be replaced with improved parts. The inspection and repair will be performed at no cost to you.

Mazda Motor Corporation ha determinado que existe un defecto relacionado a la seguridad en ciertos vehículos de motor CX-5 ano modelo 2016 producidos desde el 3 de noviembre de 2014 hasta el 7 de diciembre de 2015.

¿Cual es el problema?

Las luces diurnas {DRL} de tipo LED pueden parpadear o no iluminarse mientras conduce. Si uno o ambos DRL no se iluminan, la visibilidad del vehículo puede reducirse a otros conductores en la carretera, lo que aumenta el riesgo de un choque. Su concesionario Mazda inspeccionara la operación de DRL en su vehfculo CX-5. Si se observa una condición de parpadeo o falta de iluminación en el DRL, la(s) unidad(es) de luz averiado se reemplazaran con un nuevo conjunto de luz delantera.

Si el DRL exhibe una operación normal, se inspeccionara la fecha de producción y el numero de lote de las luces para confirmar si estan afectadas por la campaña del mercado. En las unidades de luces afectadas, se instalaran materiales desulfurantes y las juntas de sellado se reemplazaran con piezas mejoradas. La inspección y reparación se realizaran sin costo alguno para usted.

 

¿Cuanto tiempo tardara la reparación?

El tiempo requerido para la reparación dependera de la inspección. Si su vehfculo no esta equipado con las unidades de luces afectadas y solo es necesaria la inspección, se requerira aproximadamente media hora para completar la inspección. Subject: Launch of Safety Recall 4320A – 2016MY Mazda CX-5 Daytime Running Lights May Fail

 

Mazda Dealer Partners,

We realize these are challenging times and we are trying to minimize any distractions as you work to serve customers and keep your operations going as efficiently as possible. We hope this safety recall action does not add to the burden you are facing at this time and we are here to help and support you in any way we can. Please let us know if you have any concerns or need any assistance with this safety recall.

Mazda Motor Corporation has decided to conduct a Safety Recall Campaign on certain 2016 Mazda CX-5 vehicles within the VIN range below, affecting 36,761 U.S. and U.S. Territory vehicles.

Concern Outline:

The LED Daytime Running Lights (DRL) on certain subject vehicles may unexpectedly flicker or fail to illuminate while driving. If one or both DRLs fail to illuminate, surrounding traffic and pedestrians may not be able to see the affected vehicle, especially when the headlights are off in low light conditions. Please note this recall only affects CX-5 vehicles equipped with LED headlamps. Vehicles equipped with Halogen headlamps are not affected by this recall.

Due to an inappropriate material specification of the sealing gasket attached to the LED daytime running light (DRL) wiring connector, the gasket can release sulfur which may corrode the LED circuit controlling the DRL. If corrosion of the circuit develops, the circuit may lose continuity, causing flickering and/or failure of DRL illumination. One or both front DRL’s on affected vehicles could be affected by this defect.

 

For all subject vehicles:

Inspect the headlights and, if needed, replace each with an improved sealing gasket and attach desulfurizing materials (gas adsorption sheets) inside the headlight housing or if the headlights are inoperable, replace the headlight unit(s).

 

Owner Notification:

Mazda will notify owners of affected vehicles by first class mail tomorrow, March 20, 2020. Owners will be asked to bring their vehicle to the nearest Mazda dealership for repairs. As advised March 17, 2020 customers will submit their reimbursement claim for repairs that were made prior to the recall launch online at the new Mazda Customer Reimbursement website www.mazdareimbursement.com. Effective tomorrow, this recall will be tracked on the Missed Recall report.

 

Important Safety Notice:

The National Traffic and Motor Vehicle Safety Act prohibits the delivery of any subject vehicle without performing the necessary repair for defects or failures. Failure to perform applicable recalls before delivery can result in extensive dealer fines and penalties from the Federal Government. Therefore, you must complete this recall for all affected vehicles in your inventory prior to delivery.

To help you effectively perform this recall, Mazda has developed the following resources:

Due to higher than expected headlamp unit failure, combined with shipping delays created by the Covid- 19 pandemic, MNAO is experiencing headlamp unit parts supply shortages associated with this recall. If a customer has made or asks about making an appointment for repairs please know that the DRL headlights, if required, may not be available.

 

Dealer Action Required 

  • Headlamp units for this recall are currently on backorder as of April 15.
  • If a customer contacts your dealership to schedule an appointment, please ask the customer if they can check if both headlamp DRLs illuminate (Must be daylight, headlamps in Auto or Off position, Vehicle in Drive and moving, second person observes operation).
  • If the customer can confirm both headlamp DRLs illuminate properly, schedule the vehicle in for the repair.
  • If neither DRL or only one DRL illuminates, please advise your customer that parts to replace the headlamp unit are currently on backorder and offer to re-connect with your customer in 30 days to give them a status update of the headlamp availability prior to their appointment It is suggested to create a customer tracking list at your dealership so once headlamp assemblies are available you can schedule them for inspection and repairs.

This recall only impacts the Daytime Running Light, so please inform the customer that both the low beam and high beam lamps will function normally while they wait for parts to become available to replace their non-functional DRL.

 

Because of the parts shortages, this recall will also be removed from the Missed Recalls tracking report. MNAO parts distribution is working to restore supply as soon as possible and will provide parts supply updates as more information becomes available.

As a follow up to the Dealer Email sent on Wednesday April 15, 2020 due to higher than expected headlamp unit failure, combined with shipping delays created by the Covid-19 pandemic, MNAO is experiencing headlamp unit parts supply shortages associated with this recall.

 

Dealer Action Required 

  • Headlamp units that have been replaced due to DRL failure may be called back for data analysis and additional inspection.
  • If your dealer has been selected to return Headlamp assemblies, please follow the shipping instructions contained in your emailed request and ship the unit(s) back as quickly as possible to Irvine along with a copy of the repair order with technician notes.
  • Headlamp units may be scrapped 30 days after the claim acceptance date as listed on your Parts Scrap report.
  • Failure to return Headlamp units upon request will result in a debit for this All requests will be made within 30 days of the claim acceptance date.
  • Please review this communication with every technician in your dealership so they are aware of the collection activities and confirm that your technicians fully understand the repair procedures of this recall.

IMPORTANT SAFETY RECALL – PARTS UPDATE

2016 CX-5 – Daytime Running Lights May Fail – Safety Recall 4320A

This notice applies to your vehicle: 20xx Mazda Modelname VIN                              Dear Mazda Owner:

 

What is the reason for this notice? 

Mazda previously sent recall notices in March 2020 to take your vehicle to the nearest Mazda dealer to inspect the daytime running lights (DRL) operation. However, COVID-19 has impacted parts supplies for vehicles requiring replacement of headlamp assemblies. Because of this issue, complete headlamp assemblies to support this recall repair will not be available for potentially 4 to 5 months.

 

What is the problem? 

The LED type daytime running lights (DRL) may flicker or fail to illuminate while driving which may reduce vehicle visibility to other drivers on the road, increasing the risk of a crash. One or both front DRLs on affected vehicles could be affected by this defect.

 

What should you do? 

If both of your daytime running lights are working properly, please contact any authorized Mazda dealer to schedule an appointment to have your CX-5 vehicle inspected/repaired as soon as possible.

 

If either daytime running lights are flickering or failing to illuminate, headlamp unit(s) cannot be replaced at this time. This recall only impacts the daytime running lights. Both low beam and high beam lamps will function normally. Turning on the low beam headlamps will provide added daytime visibility.

 

We are working to restore supply as soon as possible and apologize for any inconvenience this may cause and appreciate your patience. Once parts supplies of headlamp assemblies have been restored, another notice will be sent notifying you that the recall has fully reopened if your vehicle required a headlamp(s).

Subject: Safety Recall 4320A- 2016MY Mazda CX-5 Daytime Running Lights May Fail – Headlamp Units on Backorder – UPDATE PLEASE READ

 

Due to various COVID-19 supplier impacts, MNAO is currently experiencing a severe supply shortage of the headlamp units associated with this recall. At this time, complete headlamp assemblies are not available to support this recall. This supply shortage is the result of production and shipping delays created by the Covid-19 pandemic.

 

On May 21, 2020, MNAO will notify customers affected by this recall with a new mailing advising them that complete headlamp assemblies to support this recall repair will not be available for potentially 4 to 5 months. Even if their daytime running light(s) are already flickering or failing to illuminate, a customer will still have to wait for further notice. Customers will also be advised if both daytime running lights are working properly, to contact any Mazda dealership to schedule an appointment, and have their CX-5 vehicle inspected/repaired as soon as possible. If the vehicle’s headlamp(s) fail during inspection, headlamp replacement is still not possible. Our goal is to be transparent with customers regarding the current part situation to help level set their expectations for repair and provide insight on timing for parts availability.

 

Owners will receive a follow-up mailing once headlamp supplies are restored to adequate levels to support this recall.

 

Due to the part supply shortage, all recall claims for a headlamp assembly requiring replacement will not be accepted with a repair date after May 25. Additionally, this recall has been removed from the Missed Recalls tracking report.

 

Dealer Action Required

  • If a customer contacts your dealership to schedule an appointment, confirm with the customer that both headlamp DRLs illuminate (Must be daylight, headlamps in Auto or Off position, Vehicle in Drive and moving, second person observes operation).
  • If the customer can confirm both headlamp DRLs illuminate properly, schedule the vehicle in for the (part number: KAYL-51-0A2)
  • If neither DRL or only one DRL illuminates, parts are not available to perform the recall. This recall only impacts the Daytime Running Please inform the customer that both the low beam and high beam lamps will function normally, and customers can utilize their low beam lamps to provide additional daytime visibility.

 

  • Note: All headlamp replacements are to be held for the required 30-days after the claim is accepted for Many dealers will see parts requests for quality and failure analysis inspections.

Headlamps not available for inspection will be debited.

 

  • Previous inspection Recall 4320A result:
    • Headlamp assemblies that light up normally even under extended strenuous testing
    • Headlamp assemblies built after the recall lot number production date

 

  • mazdarecallinfo.com will contain information about this recall that you can provide to the customer in the event they have more questions. Customers will have to input their VIN to pull up the specific 4320A recall information

 

  • The Parts Operations Division will send a communication on May 20 regarding disposition of the existing backorders and directions for placing orders for the headlamp assembly in the interim for body shop or customer pay. Only body shop or customer pay orders will be accepted until headlamp supplies are restored.

 

Please be assured the headlamp supplier and Mazda are working to increase supply as soon as possible and will provide a parts supply update as more information becomes available.

Chronology of Defect

Daytime Running Lights may mot illuminate on MY 2016 CX-5

March, 2015: The LED headlight unit, which was installed on vehicles for Japan market, did not illuminate in the vehicle manufacturing testing process. As a result of investigation, an LED circuit lost continuity due to damage from static electricity in the parts manufacturing process. Mazda determined this issue would not occur frequently but began to monitor for occurrences in the field. Mazda improved the LED component to enhance static electricity resistance properties.

 

April, 2015: Mazda received first field information from Japan market regarding the warning light illumination of adaptive LED headlight. This adaptive LED headlight is not installed on CX-5 models sold in the U.S. Mazda collected the concerned headlight unit and began investigation.

 

July, 2015: As a result of investigation on the returned parts, Mazda found it showed a similar breakage of the LED headlight as that which occurred in the manufacturing process. Mazda believed the LED headlight had been damaged by static electricity in the parts manufacturing process and resulted in failure during usage.

 

December, 2015: Parts supplier changed the original gray LED connecter sealing gasket to a low sulfur black gasket, in order to improve a separate cosmetic issue which affected the appearance of the headlight housing. At this time, Mazda and parts supplier had not been aware that this change could eventually become a permanent countermeasure for DRL functional failure.

 

March, 2016: Mazda received two field reports from outside of Japan and undertook parts investigation.

 

June through July, 2016: Mazda found that the returned parts from outside Japan were broken in a similar way to the headlight failure which occurred during the manufacturing process. Mazda confirmed failure on all returned parts from the field occurred on headlights manufactured before static electricity resistance improvement on March, 2015.

 

September, 2018: As some failures had been occurring on improved parts, Mazda investigated the possibility that this failure could occur by some other cause which was not yet known.

 

January, 2019: After further investigation, Mazda found that silver sulfide may cause damage on the LED circuit, and proximity to a certain high sulfur sealing gasket (gray type) attached to the DRL connector in the headlight unit could introduce sulfur to the LED circuit.

 

March through July, 2019: Japan authority requested Mazda to consider field action for this failure, and Mazda had several technical discussions with the authority. Japan authority determined that headlights with this failure could not comply with Japan’s vehicle regulations.

 

October, 2019: Mazda began to consider whether to conduct field action in other markets.

 

January 30, 2020: Mazda held a Quality Audit Committee to review all available information to date and determined to conduct a proactive recall action on certain MY2016 CX-5 vehicles in the U.S.
