A seat that slides forward during a crash can transfer excessive force onto a belted occupant, increasing their risk of injury.

Summary
Mazda North American Operations (Mazda
) is recalling certain 2020-2021 CX-9 vehicles equipped with 60/40 split bench second-row seats. The left-side second-row seat may unexpectedly slide forward during a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 210, “Seat Belt Assembly Anchorages.”
Remedy
Dealers will replace the internal parts of the affected second-row seats, free of charge. Owner notification letters were mailed November 4, 2021. Owners may contact Mazda customer service at 1-800-222-5500, Option 4. Mazda
’s number for this recall is 5221I.

Chronology :
January 26, 2021: Mazda was informed of this regulatory non-compliance issue by the seat supplier.
End of January, 2021 through early February, 2021: Mazda and the seat parts supplier started an investigation and identified the root cause of this issue.
February 9, 2021; The seat supplier adopted corrective measures to fix the defect in mass production.
March, 2021through August, 2021; Mazda was considering the remedy method to fix vehicles in the field which may be affected by this defect. Also, Mazda
and the parts supplier started to prepare improved parts and determined the subject population of affected vehicles with this defect in the field. Mazda
had not received any field reports of this defect from any market to this date.
September 14, 2021: Mazda held a Quality Audit Committee meeting to review all available information to date, and out of an abundance of caution, determined to conduct a proactive field action on certain MY2020 and MY2021 CX-9 vehicles in the U.S. No accidents, injuries or deaths have been reported as a result of this defect.

November 3, 2021 Attention: Mazda General, Parts and Service Managers Subject: Launch of Safety Recall 5221I – MY2020-2021 CX-9 Second Row Seat May Slide Unintentionally Dear Mazda Dealer Partners, Mazda Motor Corporation has decided to conduct a Safety Recall impacting certain MY2020-2021 CX-9 vehicles. Please see the description, model, year and VIN Ranges below. Subject Vehicles: Affects 8,648 US and US Territory vehicles: Model Subject VIN range Subject production date range 2020-2021 CX-9 With 60/40 split 2nd row seats only JM3 TC**** L0 421313 – 424632 JM3TC****M0500010 – 520573 From March 18, 2020 through February 12, 2021 Concern Outline: On certain 2020-2021 CX-9 models the driver side 60/40 split-bench second row seat may slide forward during a vehicle collision. In this condition, the seat belt may transfer a higher force to the occupant, increasing the chance of injury. There is no warning or indication to vehicle users before this defect occurs. Vehicles equipped with second-row captain seats are not affected by this recall. For all subject vehicles: On models equipped with a 60/40 split second row seat, the dealer will install modified parts in the second-row seat track mechanism. Owner Notification: Mazda will notify owners of affected vehicles for this campaign on November 4, 2021. Vehicles will display in eMDCS as Open, November 4, 2021.
